START UP Nutrition package
This program will help you get on the road to us working together towards your wellness goals. This program includes:
Intake consultation – 60 min
This initial consultation will discuss your main health concerns, discuss your dietary patterns and lifestyle habits, discuss your goals, and together we make a plan for action.
Personalized Health Plan Follow up – 30 min
This follow-up consultation will review your personalized health plan which will include a comprehensive diet plan, lifestyle recommendations and suggested supplements to support your wellness goals. This will include a personal 7-day meal plan, constructed to support your personal goals which includes full recipes and a shopping list to get you going!
TAKE CHARGE Nutrition package
This program is for those that are committed to a robust process about working together towards your wellness goals. This program includes:
Intake consultation – 60 min
This initial consultation will discuss your main health concerns, discuss your dietary patterns and lifestyle habits, discuss your goals, and make a plan for action. Subsequent follow ups need to be completed within a 90 day time frame.
Personalized Health Plan Follow up 1 – 30 min
This follow-up consultation will review your personalized health plan which will include a comprehensive diet plan, lifestyle recommendations and suggested supplements to support your wellness goals. This will include a personal 7-day meal plan, constructed to support your personal goals which include full recipes and a shopping list to get you going!
Follow up 2 – 30 min
This program is designed to help you with additional support, address any ongoing issues or updates your wellness plan based on the first changes we made together in our first sessions. This second follow up will include an additional 7-day meal plan based on the feedback you provide after your first session.
Follow up 3 – 30 min
This third follow-up session is designed to update your wellness plan, check in on your progress and discuss the changes made in the first two sessions. This third session will include an additional 7-day meal plan based on your preferences.
